Goals & progress
Goal Progress Tracking for I/DD and Adult Day Programs
“Made progress” is more useful when a team can explain which skill, what support was provided and what was actually observed.
Person-centered goals linked to everyday support
KinNotice's goal tracking design allows authorized caregivers and organizations to define objectives, starting context, an appropriate measurement and a target. Goals may involve daily living independence, communication, social participation or another person-defined outcome.
The goal record should distinguish active, paused, completed and discontinued states so that teams do not confuse an old target with the current support plan.
Observations are not the same as measurements
A staff note saying “needed fewer prompts today” provides valuable context, but it is not automatically equivalent to a formal measurement. “Completed four of five steps independently” can provide a structured measurement when it matches an approved goal model.
Keeping both record types distinct prevents summaries from overstating progress. A trend is only meaningful when the measurement definition is consistent and sufficient observations exist.
Connect goals to logs and sessions
A DSP might choose a goal-related tag during a daily log and then select the authorized goal it relates to. In a scheduled service session, staff could document planned goals at the beginning and actual observed progress at the end.
Supervisors can then review appropriate progress histories with context instead of reconstructing information from separate notebooks, spreadsheets and session notes.
A participation example
An adult day participant chooses an art activity and practices requesting materials. The staff member records the activity, available communication supports and, where relevant, an approved measurable goal outcome.
The person's choice and preferences remain part of the record. Goal tracking should not reduce participation to a compliance checkbox.
Questions people ask
Will everyday tags count as formal goal progress?
Not automatically. KinNotice's approach distinguishes general observations from defined measurements.
Does KinNotice verify therapy effectiveness?
No. The product is designed to organize documentation, not establish clinical efficacy or treatment outcomes.
