KinNotice for professional care teams
A practical guide to clearer DSP documentation
Useful documentation explains what happened, what support was provided, and what changed. It should be specific, respectful, timely and relevant to the person's goals.
What makes a daily support note useful?
Organizations should follow their own documentation policies, funding requirements, privacy obligations and clinical protocols. KinNotice is not a substitute for a certified EHR or required medication administration records.
A simple daily note framework
- Record the date, time and setting
- Describe observable activity or behavior without assumptions
- Document the support or intervention provided
- Note the outcome and any follow-up
- Connect observations to relevant goals when appropriate
